Affiliation Agreement Process

Process

Purdue University, Purdue University Fort Wayne, and Purdue University Northwest departments should use the Affiliation Agreement Intake Form on this page to initiate the process for legal review of clinical affiliation agreements. In cases where the Purdue template is used without modification, your agreement will be reviewed and, if approved, routed for signature by the authorized signatory within two business days. An OLC attorney will be assigned to review any agreements provided by a clinical site. You will receive notification of the attorney assignment and the target response date within one business day. The standard time for initial attorney review is seven business days.

The requesting department is responsible for communication between Purdue and the site, except in cases where legal counsel for the site is involved.
